EFFIE DAVIS WEIR RN-BSN- MCC is the Lead State of Texas Board Certified Case Manager for HL3CC. A nurse advocate for children and pregnant women with over 25 years of healthcare.

Case Manager Services

HL3CC receives all referrals and triage within 72 hours: the case management process consists of the comprehensive visit must be completed with the person’s parent or legal guardian. The comprehensive visit must include the completion of a Family Needs Assessment (FNA) and a Service Plan (SP). ABA Therapy Referrals Available.
